My Approach to Care
Physical therapy services are provided within my professional scope of practice. When medical evaluation or treatment is needed, patients may be referred to appropriate medical providers.
Your membership includes ongoing education, lifestyle guidance, and support.
I do not practice medicine or diagnose medical conditions
Insurance & Medicare
Your membership “Exam Services” are not covered by insurance.
These fees cannot be submitted to Medicare or insurance for reimbursement.
If medically necessary services are provided that qualify for insurance billing, those services may be addressed separately when appropriate.
Membership & Fees
Membership is paid directly by the patient
The first initial assessment fee is non-refundable and applied to membership
The agreement renews annually unless canceled within 30 days of notice
Membership fees are designed to qualify for eligibility to use HSA, FSA, HRA, or similar funds, or via employer funding, but this qualification is not assured or promised.
Integrative / Hormone / Functional Testing (If Applicable)
Your membership may involve education around integrative care.
Your licensed provider should manage any prescriptions.
There are risks and no guarantees of outcomes

How Membership Care Works
We start with a comprehensive evaluation
We create your personalized care plan
Your visit frequency is guided by your needs and goals
You receive ongoing support, adjustments, and access between visits
Your care evolves as your body and goals change
Your membership allows for care between visits, including reviewing labs, adjusting your plan, progressing exercises, and providing guidance to support your progress.
Phases of Care
Care is delivered based on your individual needs and may vary over time:
Active Phase: More frequent visits may be recommended to address specific concerns
Progression/Build Phase: Visits begin to space out as you improve
Maintenance Phase: Occasional visits with continued support between sessions
This model allows for more support when you need it and less when you don’t, while maintaining continuity of care. Many patients benefit from more frequent visits early in care. This model allows that level of support without additional cost.
This is not a pay-per-visit model
Your membership provides ongoing access to care, guidance, and resources. Visit frequency is personalized based on your needs, not a fixed number of sessions.
Care is provided as clinically appropriate and based on ensuring all members receive high-quality access.
Non-refundable initial fee of $425 covers an initial assessment, routine diagnostic exam/communications, and is applied to membership fee
What Your Membership Supports
Ongoing clinical decision-making and care planning
Lab review, tracking, and interpretation
Adjustments to your program between visits
Direct communication and guidance
This allows care to happen between visits - not just during them
Billing
Your monthly membership fee remains the same regardless of how often you are seen in a given month, as it reflects ongoing access, care planning, and support, not individual visits.
Some months may involve more frequent visits during active phases of care, while others focus on progress, integration, and support between sessions.

Cancellation
After that exam, either patient or practice may terminate the subscription with no further charge, or elect to continue with ongoing care that we know statistically results in a superior care experience.
Appointment Policy
Late cancellations or missed visits may result in $150.00 fee
