Our Membership Model
Your health is always changing, and some journeys take time.
Membership means you don’t have to navigate those changes alone. Together we keep asking questions, connect the dots, and help you find the next step.
This model gives you the continuity of working with a physical therapist who knows your goals and where you are in your journey. It creates space for ongoing support and individualized care as your needs and body change over time.
See below for some of the other benefits.
Pelvic and whole-body physical therapy as appropriate
Functional-health support
Individualized exercise and health recommendations
Communication and support between visits
Review of relevant labs, records, and health information when appropriate
Help navigating referrals and other healthcare providers
Ongoing assessment as symptoms and goals change through life transitions
Before and after support surrounding pregnancy/postpartum status, hormones, activity, surgeries/procedures, or health circumstances
Continuity with one clinician who knows your history
Movement through active care, stabilization, and long-term maintenance rather than repeatedly starting over or separating body parts and the flexibility to go in and out of these phases depending on your needs.
Membership is $650.00/month and care plans typically last anywhere from three to six months.

Obsidian Insight is designed for continuity, not one-off visits.
Membership gives us the time and flexibility to address what is happening now, follow how your body responds, adjust your plan, and support you as your needs change. Instead of repeatedly starting over, we build on an ongoing understanding of your health, goals, and history.
-
Membership may include pelvic and orthopedic physical therapy, hands-on treatment, movement and exercise guidance, whole-body health assessment, review of relevant labs and health information, personalized recommendations, support between visits, and coordination with other healthcare professionals when appropriate.
Your care is individualized, so not every member uses every part of the membership in the same way.
-
Visit frequency is based on what you need rather than a preset treatment schedule.
You may need more support during an active problem, pregnancy, postpartum recovery, or a flare, and less as you move into stabilization, strength building, or maintenance.
We will discuss the appropriate frequency together.
-
No. Your first visit is an opportunity to understand what is going on, determine whether Obsidian is the right fit, and discuss what type of ongoing support would make sense.
You will know the recommended next steps before deciding how you would like to proceed.
-
Membership is designed for people who want greater continuity, flexibility, access, and a broader approach to their care. It allows us to respond to changes without treating every new concern like a completely separate episode.
-
That is expected. Health is rarely linear. Some months may require more attention, while others may focus on maintaining progress, building strength, reviewing new information, or planning ahead.
Membership is designed around an ongoing care relationship rather than trying to make every month look identical.
-
Between visit support may include brief questions, clarification of recommendations, updates about how you are responding, and guidance about whether something should be addressed sooner or at your next appointment.
It is intended to help you stay connected to your plan without feeling like you have to wait until your next visit to ask every question.
-
No. Membership provides ongoing access to Obsidian Insight within the scope of the services we provide. It does not replace primary care, urgent care, emergency care, or specialist medical care when those services are needed.
-
Services are set up to qualify for HSA and FSA use. Eligibility depends on your plan and the services provided, so you should confirm with your plan administrator when needed.
-
Membership tends to work best for women who value continuity, want a clinician who understands the larger picture, and prefer a relationship-based approach rather than managing each symptom or concern separately.
It is especially helpful when needs span pelvic health, movement, pregnancy or postpartum, midlife changes, or broader whole-body health concerns.
